About the Role
We are currently hiring for a Full Time Registered Nurse position in the Framingham and Marlborough areas. As a Nurse Care Specialist, you will be responsible for providing and directing provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

Responsibilities

• Provides comprehensive nursing evaluations and develops individualized plans of care in collaboration with the patient, family, and healthcare team.
• Coordinates total plan of care with other healthcare professionals involved in care and helps to achieve and maintain continuity of patient care.
• Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.
  Requirements
  
  • Current RN licensure in the state of MA
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
